This post will accumulate rock-climbing diagnoses:

fractured talus (A.Y. Jun 2012)

fractured ankle (S.Y. Jul 2012) - was leading "5.8", as usual lost way, ending up on a route of unknown category (5.12+?), fell 2-3 meters.

proximal phalanx annular pulley "issues" (P.B. Aug 2012)  - bouldering after a long break (Update: Back to climbing Sept 2012)

Honorable mention:
fractured distal radius with dorsal displacement (Colles' fracture), (A.G. Aug 2012) - Hang gliding, hit a huge descending "anti-thermal" while on approach to the  airport, fell on the roof of two-story house, then from said roof. Contributing factor - failure to maintain proficiency & currency due to insufficient hours of flight time per month.
